1997 Grand Prix GTP Tranny issues?

My Pontiac Grand Prix GTP Supercharged seems to be shifting hard/funny. Sometimes you have to lift off the throttle a little bit to get it to shift, it seems to jerk a lot at highway speeds and when going up a grade it tends to drop into a lower gear and makes a weird clicking sound sometimes accompanied by the jerking as well. Is the transmission screwed or is there hope?
